Tell us about your form
When you need a form built — a contact form, an application, a registration, a survey — the more you can tell us up front, the faster we can build exactly what you need. You don’t need answers to everything; share what you know and we’ll fill in the gaps together.
What’s the form for?
A sentence or two on what it should do and who fills it out.
(e.g. “A volunteer sign-up for our spring event,” or “A contact form for our services page.”)
What information should it collect?
For each field you want, tell us:
- The label the person sees — “Full name,” “Email,” “Phone,” etc.
- Whether it’s required or optional
- The kind of answer — short text, a longer message, email, phone, a dropdown or checkboxes (list the choices), a date, a file upload…
Where should responses go?
- Which email address(es) should be notified on each submission?
- Should we also save responses to a spreadsheet, or send them to your CRM or mailing list (e.g. Mailchimp)?
- Should the person who submitted get an automatic reply or receipt?
What happens after someone submits?
- A thank-you message on screen, or send them to a specific page?
- Any wording you’d like for the confirmation.
Anything special?
- File or photo uploads (roughly how large)?
- Collecting a payment or donation through the form?
- Questions that appear only based on an earlier answer (conditional logic)?
- A consent or privacy checkbox?
- Which page on your site the form should live on.
